Your appointment is very important. We understand that sometimes schedule adjustments are necessary. Therefore, we respectfully request at least 24 hours notice prior to your scheduled appointment time for cancellations or rescheduling of appointments.
Please notify us by e-mail at contact@evolvemedicalcare.com or by using the Calendly booking system where you created the appointment.
ANY APPOINTMENTS CANCELLED/RESCHEDULED/CHANGED WITHOUT 24 HOURS NOTICE WILL RESULT IN A $100 CHARGE.
ALL “NO SHOWS” WILL BE CHARGED $100. FEES MUST BE PAID BEFORE REBOOKING.
AFTER 3 NO SHOWS YOU MAY BE DISCHARGED FROM THE PRACTICE.
We recognize the time of our clients and provider is valuable and have implemented this policy for this reason. When you miss an appointment with us, we not only lose your business but also the potential business of other clients who could have scheduled an appointment for the same time.
ARRIVAL TIME
Please arrive at your appointment at least 10 minutes prior to your scheduled appointment time. All services have a specific time schedule. An early arrival allows for a relaxed experience. If you arrive late, your service(s) may be shortened in order to maintain the clinic’s schedule.
LATE ARRIVAL POLICY
All appointments begin and end on time in order to maintain the clinic’s schedule. If the appointment does not start on time due to client tardiness, the appointment time will be reduced accordingly. If a client is more than 10 minutes late, the appointment will be considered a same day cancellation.
